Free Medical Treatment for Senior Citizens

As per the unverified information available in public domain, many developed and developing countries have been providing either free medical treatment including medicines and hospitalisation or on subsidised cost, to their senior citizens. However, specific details of such schemes/policies are not available with the Health Ministry.

The Government of India had launched National Programme for Health Care of the Elderly (NPHCE) during 2010-11 with a view to provide separate and specialized comprehensive health care facilities to senior citizens at various levels of state health care delivery system including outreach services. The health care facilities, being provided under this programme, are either free or highly subsidised.

The major activities of the NPHCE are:-

i) Setting up of Geriatric department in identified Regional Geriatric Centres (RGCs) with OPD care services and 30 bedded Geriatric ward for providing indoor services. RGCs will also undertake PG Courses in Geriatric for developing Human Resource

ii) Setting up of 10 bedded Geriatric units at all District Hospitals that will provide indoor services for elderly during hospitalisation.

iii) Establishment of Rehabilitation unit at all Community Health Centres (CHCs) and Geriatric clinic twice/week.

iv) Setting up weekly Geriatric clinic by trained medical Officer at Primary Health Centres (PHCs).

Apart from this, Government of India has launched a Senior Citizen Health Insurance scheme with effect from 01.04.2016, as a top up scheme of Rashtriya Swasthya Bima Yojna (RSBY). The scheme will provide health coverage to senior citizen who are aged 60 years and above belonging to BPL category. Under this scheme, the health coverage is for Rs. 30,000/- per annum per senior citizen for treatment packages, over and above RSBY entitlement. The scheme would be cashless and run on the RSBY platform as an add-on cover. The add-on cover of Rs. 30,000/- per senior citizen will be for treatment of senior citizen only. Senior citizen are also eligible for cover under the primary RSBY on family floater basis of Rs. 30,000/-.

It is further added that Public Health is a State subject Under the National Health Mission, support is provided to States/UTs for provision of drugs free of cost to all those who access public health facilities including the elderly population.

The Health Minister, Shri J P Nadda stated this in a written reply in the Rajya Sabha here today.

Self-Printing of CGHS Card

F.No.Z.15025/1/2016/DIR/CGHS
Government of India
Ministry of Health and Family Welfare
Directorate General of CGHS

Nirman Bhawan, New Delhi – 110011
Dated: 31st March, 2016

OFFICE MEMORANDUM

Subject: Self-Printing of CGHS Card

Kindly refer to the above subject.

2. This is to intimate that a facility for self-Printing of CGHS cards from CGHS website (cghs.nic.in) has been launched by the Hon’ble Union Health Minister on 29th March, 2016. This facility, which is available only to those beneficiaries who are already a part of the CGHS, will prove to be useful in various situations.

3. The basic purpose of this reference is to highlight that a card self-printed by the beneficiary stands on the same footing as one printed and issued by the CGHS itself. Software has been designed to ensure that the self-printed card will be of the same colour as the card issued by the CGHS. Therefore, all the staff working in your Hospital/Clinic/diagnostic Centre may be sensitized to ensure that there is no harassment to a beneficiary carrying a self-printed card. In case of any doubts. the particulars on the self-printed card may be compared with the CGHS database already available on CGHS website. Such instances should however be rare.

“A significant steps towards Digital India and for easy access of health services to the common man”: Shri J P Nadda

Press Information Bureau
Government of India
Ministry of Health and Family Welfare

Shri J P Nadda launches GIS-enabled HMIS application, self-printing of e-CGHS Card

“A significant steps towards Digital India and for easy access of health services to the common man”: Shri J P Nadda

Shri J P Nadda, Union Minister of Health and Family Welfare launched two major Digital initiatives to provide easy access to health services to the public: GIS-enabled HMIS application and the self-printing of e-CGHS cards, here today. Launching these initiatives of the Health Ministry, the Union Minister stated that the Health Ministry remains committed to strengthening public health systems and providing user-friendly health services in the country. He further stated that that this will provide a tremendous opportunity to further strengthen and enhance the reach of key interventions.

Terming these as significant and forward looking steps contributing to Digital India, Shri J P Nadda said: “The initiative will not only monitor the performance and quality of the health services being provided under the NHM, but will also provide effective healthcare to the rural population throughout the country”. The GIS-enabled HMIS services will provide comprehensive data on a GIS platform on 1.6 lakh Government Health facilities spread across the country. The project has been developed with the objective of enabling the effective usage of the HMIS application, enhancing analytical capabilities of States/UTs and providing a geographical display of HMIS data using GIS. The application also has scope of incorporating map layers of roads and water bodies which will provide comprehensive information regarding health facilities and disease vulnerable areas of the country. At present, the application is available in login domain of HMIS portal. It is planned to put it in public domain in near future. Moreover, the mapping provides data only on public health facilities presently, which shall soon be expanded to cover private facilities also, the Union Minister informed.

Health Management Information System (HMIS) is a web-based monitoring system that has been put in place by Ministry of Health & Family Welfare (MoHFW) to monitor its health programmes and provide key inputs for policy formulation and interventions. It was launched during October, 2008 with uploading of district consolidated figures. To make HMIS more robust and effective and in order to facilitate local level monitoring, “facility based reporting” was initiated since April, 2011. At present, 1.94 lakh health facilities (across all States/UTs) are uploading facility wise data on monthly basis on HMIS web portal. Data are presently being made available to various stakeholders in the form of standard & customized reports, factsheets, score- cards etc. HMIS data are widely used by the Ministry and States for policy planning, monitoring and supervision purpose.

The Union Health Minister, while launching the self-printing of the E-CGHS card, stated that this will provide ease and access to all those beneficiaries such as the pensioners who may find it difficult to go to the CGHS centres for renewal, and in the event of loss/misplacement of the card. As this can be printed from any location once the process of application based on authentic documents has been approved, this enables easy reprinting by the beneficiary himself/herself. The beneficiary can access this facility by visiting the CGHS portal at cghs.nic.in where simple steps for this have been enumerated.

Also present at the meeting were Shri B P Sharma, Secretary (HFW), and senior officers of the Health Ministry.

Linking of CGHS Card number with Aadhaar is purely on a voluntary basis

The below information was given in written reply to a question about the linking between CGHS card and Aadhaar by the Minister of Health and Family Welfare Shri Jagat Prakash Nadda in Lok Sabha on 11th March 2016 as follows…

“Ministry of Health and Family Welfare has issued an Office Memorandum No. Z-15025/11/201-DIR CGHS dated the 10th July 2015 requesting the Ministries/Departments concerned to motivate their employees availing CGHS to link their Aadhaar Numbers with the CGHS database either online themselves through the CGHS portal or through the CMO-in-charge of their dispensary.

Central Government employees/pensioners applying for new CGHS cards/new plastic cards have the option to provide Aadhaar Number for entering the number in CGHS database.

Linking of CGHS beneficiary ID number with Aadhaar number is purely on a voluntary basis.

Linkage of Aadhaar number with CGHS ID will facilitate consolidation of all health-related data bases and enable more convenient access to various E-services in the future. “

Health Insurance Scheme - 7th CPC Recommendations

In this backdrop, the Commission opines that health insurance for the government employees and pensioners remains the most optimal route for ensuring complete coverage for all employees, pensioners and their dependants in the long run. The IV CPC had suggested that feasibility and modalities of an Insurance Scheme for government employees in lieu of medical reimbursement may be considered by the government. The VI CPC had recommended introduction of a health insurance scheme for Central Government employees and pensioners.

It had recommended that for existing employees and pensioners, the scheme should be available on a voluntary basis, subject to their paying the prescribed contribution. It had also been recommended that the health insurance scheme should be compulsory for new government employees who would be joining service after the introduction of the Scheme.

Similarly, it had recommended that those retiring after the introduction of the insurance scheme would be covered under the Scheme.

9.5.16 The Commission observes that in view of the recommendations of the earlier Pay Commissions and various high power committees, the government has been contemplating the introduction of a health insurance scheme on Pan-India basis. The Commission notes that although the Committee of Secretaries had given its ‘in principle’ approval way back in 2011,and an amount of ? 2,061 crore had been earmarked under the XII Five Year Plan, the Scheme has still not been implemented.

9.5.17 The Commission notes that given the tardiness in the introduction of the long awaited Insurance Scheme, as already mentioned earlier in this chapter, the pensioners residing outside CGHS area will continue to be at a disadvantage, in terms of medical facilities, compared to their counterparts residing in CGHS areas. As stated earlier in this chapter, according to existing provisions, pensioner residing outside CGHS area but subscribing to CGHS for OPD/IPD can avail medical facility from any hospital–private or public or empanelled under CS (MA)/ECHS–in his/her own city. However, in such cases, the pensioners have to make upfront payment to the hospital and claim reimbursement later. The Commission feels that this could be a limiting factor for many pensioners who may not have the resources to pay hospital expenses upfront. The Commission notes that under CS (MA) Rules/ECHS, there are empanelled hospitals in every part of the country, at least in all major locations. In this backdrop, after identification of some major centres/cities based on minimum population threshold of pensioners, these hospitals could be empanelled by CGHS as well, for extending medical facilities on a cashless basis.

Considering all the issues, the Commission makes the following recommendations:

i. The Commission strongly recommends the introduction of health insurance scheme for Central Government employees and pensioners. In the interregnum, for the benefit of pensioners residing outside the CGHS areas, the Commission recommends that CGHS should empanel those hospitals which are already empanelled under CS (MA)/ECHS for catering to the medical requirement of these pensioners on a cashless basis. This would involve strengthening of administrative capacity of nearest CGHS centres. However, this step will go a long way in ameliorating the pending grievances of these pensioners.

ii. The Commission recommends that the remaining 33 postal dispensaries should be merged with CGHS. The Commission further recommends that all postal pensioners, irrespective of their participation in CGHS while in service, should be covered under CGHS after making requisite subscription.

iii. Currently, there are various health care schemes in the Central Government catering to specific sets of employees. For example, apart from CGHS, there are Ex-Servicemen Contributory Health Scheme (ECHS) and Railways Employees Liberalized Health Scheme (RELHS) which cover ex-servicemen and Railway employees/pensioners, respectively. Although the patterns in these schemes vary, a combined entity of CGHS-ECHS-RELHS would result in a very strong network of health facilities for the Central Government employees across the length and breadth of the country. The Commission  recommends that possibility of such a combined network of various medical schemes should be explored through proper examination.

NO CGHS Funds for next 2 Months

Health Ministry – Started in 1954, the CGHS — with approximately 30 lakh beneficiaries — covers retired and serving central government employees, MPs and their immediate families across 25 Indian cities.

The health ministry has no money left in the Central Government Health Scheme (CGHS) account for the next two months till funds according to the revised estimate are released. Having exhausted the Rs 1,600 crore that was initially allocated, the ministry has now asked for Rs 450 crore more.

The crisis in CGHS, one of the oldest health schemes run by the government, was precipitated by successive budget cuts, said sources.

Started in 1954, the CGHS — with approximately 30 lakh beneficiaries — covers retired and serving central government employees, MPs and their immediate families across 25 Indian cities. A total of 945 private hospitals and 165 diagnostic centres are empanelled with CGHS, which means that treatment in these hospitals for specified conditions are reimbursed by the government. Expenses at government hospitals are automatically covered for CGHS card holders.

“Funding for CGHS has been steadily going down, in 2013-14 it was Rs 1,832 crore, in 2014-15 it was Rs 1,798 crore and this year it was Rs 1,600 crore which we have exhausted. More funds will come only in January which is when the demands submitted under revised estimates are released. So the next two months will be difficult. We have asked for another Rs 450 crore for the rest of the year,” a source said. The matter was discussed at a presentation given to Health Minister J P Nadda on Tuesday, along with a host of measures to increase the beneficiary base and make transactions easier through the use of technology.

While instructions were issued to start clearing bills at one go instead of the earlier practice of 10 per cent payment in first instalment and the rest later, the ministry realises that the Rs 80-90 crore corpus that was handed out to UTIITSL for payment purposes is hardly sufficient.

“We need at least a couple of hundred crores corpus for the payments to continue without any hitch. We are looking at how to raise that money from other sources if possible,” said an official. The fund crunch hit at a time when the ministry was in the process of clearing private hospital bills to the tune of several hundred crores, pending for years. The pendency had led to a mass exodus of hospitals a couple of years ago, drastically reducing the number of private institutions empanelled with CGHS.

However, unfazed by the fund shortage, officials said this was only a “temporary phase”.
