OFFICE ORDER

Subject: Suspension of empanelment of Shanti Gopal Hospital, Indira Puram, Ghaziabad.

With reference to the above mentioned matter, the undersigned is directed to draw attention to the Office Memorandum No. S-11011/23/2009-CGHS-D-II/ Hospital Cell (Pt.) dated 07.10.2010 vide which Shanti Gopal Hospital, Indira Puram, Ghaziabad was empanelled under CGHS and to state that Shanti Gopal Hospital, Indira Puram, Ghaziabad has violated the terms & conditions of MoA signed the CGHS and has refused credit facilities to bonafide CGHS beneficiaries. A show cause notice was issued to the hospital and reply received was found to be unsatisfactory. In view of this it has been decided to suspend empanelment of Shanti Gopal Hospital, Indrapuram, Ghaziabad from CGHS with immediate effect till further orders.

The CGHS beneficiaries already admitted in the hospital for treatment prior to the issue of this order shall however be continued to provide treatment at CGHS rates
